چکیده

ObjectiveTo report a successful case of ovarian hyperstimulation and oocyte cryopreservation in transgender male adolescent after suppression with gonadotropin-releasing hormone (GnRH) agonist while using the aromatase inhibitor letrozole to maintain low serum estradiol.DesignCase report.SettingDivision Reproductive Endocrinology Infertility, Washington University St. Louis School Medicine, St Louis, Missouri.Patient(s)A 15-year-old Tanner II GnRH implant.Intervention(s)The implant was removed. The patient given (5 mg daily) undergoing stimulation an antagonist protocol. After retrieval, began taking testosterone.Main Outcome Measure(s)Successful minimal changes breast budding.Result(s)The patient's peak estradiol concentration 510 pg/mL. Twenty-two mature oocytes were cryopreserved. Small increases budding occurred between baseline time retrieval. Conclusion(s)We successfully used during stimulation. Maintaining minimize pubertal development possibly prevent gender dysphoria symptoms may make more desirable for adolescents. To estradiol. Case report. Division Missouri. A implant. testosterone. Circulating luteinizing hormone level after triggering oocyte maturation with GnRH agonist may predict oocyte yield in flexible GnRH antagonist protocol.

BACKGROUND The use of gonadotrophin-releasing hormone (GnRH) agonist for triggering final oocyte maturation and ovulation can reduce ovarian hyperstimulation syndrome (OHSS) in high-risk patients. LH levels post-trigger with GnRH agonist might be correlated with oocyte yield and maturity. Our aim was to evaluate the relationship between serum LH level at 12-h post-trigger and oocyte yield, matu...

I-27: Management of Poor Responders:Current and Past Recommended Strategy

The management of poor responders in IVF cycles is extremely controversial due to their inadequate response to controlled ovarian hyperstimulation.. A low ovarian response may be idiopathic or may be associated with many different factors including age, diminished ovarian reserve, endometriosis and prior ovarian surgery, and a reported incidence 9-24% of IVF patients. Several strategies have be...

GnRH agonist versus GnRH antagonist in oocyte donation cycles: a prospective randomized study.

BACKGROUND The specific role of LH in folliculogenesis and oocyte maturation is unclear. GnRH antagonists, when administered in the late follicular phase, induce a sharp decrease in serum LH which may be detrimental for IVF outcome. This study was performed to evaluate whether the replacement of GnRH agonist (triptorelin) by a GnRH antagonist (ganirelix; NV Organon) in oocyte donation cycles ha...

High yield of oocytes without an increase in circulating estradiol levels in breast cancer patients treated with follicle-stimulating hormone and aromatase inhibitor in standard gonadotropin-releasing hormone analogue protocols.

BACKGROUND Adjuvant/neoadjuvant chemotherapy in breast cancer patients may be associated with amenorrhea and a marked reduction in ovarian reserve. OBJECTIVES To assess the use of letrozole with follicle-stimulating hormone (FSH) in gonadotropin-releasing hormone (GnRH) analogue protocols, based on reported attempts to avoid the estradiol (E2) increase during controlled ovarian hyperstimulati...
